Physiotherapy for Men with Osteopenia and Osteoporosis

Fragility fractures in men aged over 50, while less prevalent than in women, are associated with higher morbidity and mortality. Exercise is a key modifiable factor in fracture prevention, primarily through improving bone strength, muscle capacity, and reducing falls risk.
Recent research known as the LIFTMOR-M trial has provided strong evidence supporting the role of high-intensity, targeted exercise in improving bone health for middle-aged and older men with low bone density.
The study looked at men over 45 years with osteopenia or osteoporosis and compared different exercise approaches over an eight-month period. The most effective program was a High-Intensity Resistance and Impact Training (HiRIT)program, completed twice per week under supervision.
Physiotherapy-Led Exercise Guidelines (Based on LIFTMOR-M)
Physiotherapy programs should include:

- Impact intensity gradually increased, progressing towards stiffer landings as tolerated

- Target RPE ≥16/20 on the Borg Scale
- Load prescription
Frequency & Duration
- 2 sessions per week
- Minimum 8-month program to achieve meaningful bone and functional adaptation
Participants who followed this approach showed greater improvements in bone density at the hip and lower spine, along with significant gains in muscle strength, balance, and overall physical function.
